How Is a Ligament Tear in the Knee Diagnosed Without Surgery?

A twisted knee, a sudden pop during sport, or a direct blow to the side of the leg often raises the same question soon after: is this a ligament tear, and how would anyone know without cutting the knee open to look? The reassuring answer is that surgery isn’t needed just to diagnose a ligament tear. A combination of clinical examination and imaging gets doctors to an accurate answer in the vast majority of cases. Here’s exactly how that process works.

The four ligaments involved

The knee is stabilised by four main ligaments, and knowing which one is likely involved starts with understanding what each one does.

ACL (anterior cruciate ligament). Sits in the centre of the knee and prevents the shin bone from sliding too far forward relative to the thigh bone. It’s the most commonly injured of the four, often during sports involving sudden stops, pivots, or landing awkwardly from a jump.

PCL (posterior cruciate ligament). Also central, but prevents the shin bone from sliding too far backward. Typically injured by a direct blow to the front of the shin, such as in a car dashboard injury or a fall directly onto a bent knee.

MCL (medial collateral ligament). Runs along the inner side of the knee, resisting inward-bending forces. Commonly injured by a direct blow to the outer side of the knee, which pushes the knee inward.

LCL (lateral collateral ligament). Runs along the outer side of the knee, resisting outward-bending forces. Less commonly injured than the others, usually from a blow to the inner side of the knee.

Different ligaments often get torn from different specific mechanisms, which is exactly why the story of how the injury happened is one of the first and most useful clues a doctor works with.
